To begin the process of booking an appointment, please use the link below to make a request from the available dates and times. Please note that requesting an appointment requires you to provide a valid e-mail address. After your request is submitted, you will receive an e-mail notification and will have 7 days to complete the necessary intake forms and submit payment for the session. If you do not complete the intake forms or payment in the allotted time your session request will be canceled and you will need to request a new appointment.

Note: Once a session is booked it is non-refundable. However, sessions can be rescheduled once with a minimum advance notice of 24 hours and must be rescheduled for a date within one month of the originally scheduled session.
